What is a DOT physical?
Federal Law Requires it
The Department of Transportation (DOT) physical exam is a mandatory medical evaluation required by the Federal Motor Carrier Safety Administration (FMCSA) for all commercial motor vehicle (CMV) drivers.
The exam ensures that drivers are physically and mentally fit to safely operate large commercial vehicles on public roads. If you hold or are applying for a Commercial Driver’s License (CDL), this exam is required by federal law.
Vision
Must meet 20/40 acuity in each eye, with or without corrective lenses.
Hearing
Ability to perceive a forced whispered voice in the better ear at 5 feet.
Blood Pressure
Cardiovascular screening to detect hypertension and heart conditions.
Respiratory
Lung function and respiratory health evaluation.
Urinalysis
Checks for underlying medical conditions including diabetes.
Musculoskeletal
Range of motion, strength, and physical ability to operate a CMV.
Certificate Validity
A DOT Medical Certificate is valid for up to 24 months. Certain conditions may require more frequent exams (every 12, 6, or 3 months).

Prepare for your visit
What to Bring
Having the right documents speeds up your exam and ensures accurate certification. Please bring the following:
Valid government-issued photo ID or driver's license.
List of all current medications (including dosages).
Eyeglasses or contact lenses if you use them.
Hearing aids if you wear them.
Medical records for any known conditions (diabetes, sleep apnea, heart conditions, etc.).
Previous DOT Medical Certificate (if renewing).
Documentation from your treating physician for any exemptions or waivers.
Vision – If you do not meet the minimum vision standards or having a chronic eye disease like glaucoma you need to bring to your DOT Physical exam the Vision Evaluation Report Form MCSA–5871 completed, signed and dated by your ophthalmologist or optometrist not more than 45 days before your DOT Physical exam.
Insulin Treated Diabetes Mellitus Assessment Form (ITDM), MCSA-5870, must be completed by your Treating Clinician. You can download the Insulin-Treated Diabetes Mellitus Assessment at Form MCSA-5870.
After your exam
If you pass, you’ll receive the MCSA-5876 Medical Examiner’s Certificate — the wallet-sized card you must carry while driving a CMV.
Your results are electronically submitted to the FMCSA National Registry, automatically updating your CDL record with your state DMV.
You’ll also receive a copy of the completed medical examination report for your records.

How long does the exam take?
Most DOT physicals are completed in 20–40 minutes. If additional evaluation is needed for a specific condition, it may take slightly longer.
How much does a dot physical cost?

What disqualifies a driver?
Certain conditions can be disqualifying, including uncontrolled diabetes, severe hypertension, vision below 20/40, or active substance use. Exemptions may be available — ask our examiner.
Can I be seen if I have a condition?
Yes. Conditions like controlled diabetes, sleep apnea, or high blood pressure don’t automatically disqualify you. Bring documentation from your treating physician for the best outcome.

Is the exam valid in all states?
Yes. DOT physicals conducted by FMCSA-registered Medical Examiners are federally recognized and valid in all 50 states.
